About | Patient-Centered Medical Home Let EGHC be your Patient-Centered Medical Home The patient-centered medical home is a way of organizing your care that emphasizes care coordination and communication to transform primary care into "what patients want it to be." As your medical home, we will:
Please keep in mind that our partnership with you will function ONLY if you provide us with a complete medical history and information about any care you have received outside of EGHC. Appointment / TelephoneTo schedule, cancel or change an appointment, please call the office nearest you. Please bring all current medications with you to your visit. If you are unable to keep a scheduled appointment, please call our office to reschedule so that someone else may take your place. If you arrive more than five minutes late, our policy is to give your appointment to a walk-in patient. We will still accept you when you arrive, but we will have to work you into the schedule as soon as possible. If you need to contact your provider during business hours, our office staff will take a message and you will receive a return call within one business day. Please let us know whenever you have been see in the Emergency Room or been in an in-patient hospital, as well as any medical history or information about your care outside of this practice. Same-Day Sick VisitsOur offices intentionally leave same-day openings in their daily schedule for those who are acutely ill. We prefer that you call ahead to schedule an appointment, however, we will accept walk-ins on a first-come, first serve basis. After-hours call specific clinic and calls will be forwarded to our answering service. Calls for medical advice will be returned by an EGHC provider. For emergencies that cannot wait, please call 911 or go to the nearest emergency room. |
